Social Determinants of Health (SDoH) Data in Action

Just when clinicians—inspired by the HITECH act and new forms of analytics—are grappling with new uses for traditional clinical data, they are being pushed to include a far vaster data set: social determinants of health (SDoH). There are often challenges when it comes to figuring out what types of SDoH data are relevant to each patient, and how to act on it to create healthier individuals and communities.

Health Gorilla is devoted to deriving insights from a combination of traditional clinical data and SDoH.

Avery Haller, executive director for strategy and partnerships, divides relevant data into three types:

  • Typical census and public health data, usually available at the ZIP-code level and therefore of limited use in assisting individuals.
    · Specific needs assessment data which is typically provided by the patient.
  • Third-party data about individuals, which helps to flesh out the other two.

To integrate the third type of data, Health Gorilla joined forces with LexisNexis® Risk Solutions. Access to such data helps clinicians identify where a patient might be more likely to make a visit or pick up medication if the clinic provides transportation. They can look at other items in what LexisNexis Risk Solutions calls a “motivation risk score.”

It’s important to remember that data is the foundation in which everything else builds from and having a solid base is imperative. With thousands of data points that can be applied to each individual, analytics are required to turn up what’s useful.  Historically, we never thought much about what happened before the patient came into the office or what happens after they go home.”
